Maximus is currently seeking a Lead Registered Nurse to provide outstanding home assessment services to vulnerable populations and meet position qualifications to support the New York Independent Assessor Program (NYIAP). The program serves the State of New York by conducting a UAS assessment to determine eligibility for community-based long-term services. In this role, the nurse will have the opportunity to conduct home assessments and develop leadership skills by supporting the Regional Nurse Manager and a team of Nurse Assessors. *This position is hybrid-remote and will support the Bronx, NY. The position requires attending and conducting assessments in consumer homes and through a telehealth modality.

Essential Duties & Responsibilities: - Responsible for conducting clinical appointments and precepting new clinicians - Maintain current knowledge of regulatory changes pertaining to quality and validity of assessments - Assess cases presented and utilizes clinical competencies to review and validate assessment outcomes - Work collaboratively with manager to identify areas for improvement with process and quality of care - Suggest training needs for assigned unit staff - Provide technical assistance to assigned unit staff - Support Company and project initiatives through various methods including messaging to the unit, leading assigned projects, training staff, participating in work groups, etc. - Participate in performance, improvement, and quality assurance activities and related meetings - Provide clinical and educational support to meet program goals - Meet all standards established for this position as outlined in the corresponding annual performance criteria and bonus template for this position - Perform initial in-home evaluations of patients that includes evaluation of health status, strengths, care needs, and preferences utilizing the UAS-NY assessment tool - Establish and maintains positive relationships with consumers, caregivers, and facility staff - Maintain updated knowledge of the UAS-NY Assessment process - Review consumer medical documentation and/or health referral forms, as relevant to the case - Enter evaluation data into electronic evaluation form and transmit, as required - Document any concerns, conflicting information, other issues that surface during the evaluation process - Emphasize continuity of care, thus reducing or eliminating fragmentation, duplication, and gaps in the treatment plan - Initiate communication between the Call Center and eligible consumers for enrollment into a managed care health plan - Facilitate and support new IA Nurse Evaluators to meet the essential functions of the Nurse Evaluator position - Conduct IA appointment visits with new Nurse Evaluators to provide demonstration and education - Instruct, demonstrate, observe assessment skills and techniques and provides feedback to the Regional Nurse Manager Minimum Requirements - Current Registered Nurse (RN) license valid in the state of practice is required - Two (2) or more years practicing experience as a Registered Nurse - At least one (1) year of experience in training, precepting, acting as a clinical advisor, providing unit support, working in the field independently/successfully - Must have active ID Number or be willing to obtain - Preferred work experience with the adult/geriatric populations; home care experience, experience working with individuals with physical or developmental disabilities - Required ability to travel throughout assigned region, up to 60% - 1+ years experience using the UAS-NY Community assessment tool- Active and valid New York State Driver’s License or ID- Reliable form of transportation which may include ride-sharing options Preferred Requirements - Bachelor of Science in Nursing- Prior experience MLTC, CHHA, and/or LHCSA experience- Prior experience conducting in-home assessments- Experience using Microsoft Office application- Ability to type at least 30 WPM Home Office Requirements - Maximus provides company-issued laptop and phone- Reliable high-speed internet service - Minimum 20 Mpbs download speeds/50 Mpbs for shared internet connectivity - Minimum 5 Mpbs upload speeds #NYMC #LI-Hybrid
