Now Hiring: MDS Coordinator RN – Grand Rapids, MI


Are you an experienced RN passionate about clinical excellence, team leadership, and ensuring accurate reimbursement? We’re seeking a skilled MDS Coordinator RN to lead the Resident Assessment Instrument (RAI) process at our sub-acute rehab facility in Grand Rapids, Michigan.

Position Highlights:
As the MDS Coordinator RN, you will:

  • Oversee and manage the MDS/RAI process to ensure accurate, timely assessments.
  • Supervise a team of MDS Nurses—conduct evaluations, mentor, coach, and schedule.
  • Lead interdisciplinary care planning and coordinate Medicare/insurance utilization efforts.
  • Review and analyze clinical data to optimize PDPM reimbursement categories.
  • Educate staff on documentation best practices for patient care and compliance.
  • Participate in Triple Check, QAPI, and facility leadership initiatives.

What We're Looking For:

  • Current RN license in Michigan (required)
  • Graduate of an accredited School of Nursing
  • CPR certification
  • Minimum 2 years of nursing experience, preferably in a rehab or skilled setting
  • Prior MDS experience and PointClickCare proficiency
  • Familiarity with CMS RAI guidelines strongly preferred
  • Strong communication, organizational, and leadership skills
