We are currently seeking a dedicated Mental Health Therapist to be part of our innovative Collaborative Care Model (CoCM) team. As a key player in our team, you will provide essential clinical services in a primary care environment, working alongside Optum Behavioral Care to support the mental and physical well-being of patients. This exciting role involves collaboration with both patients and healthcare professionals to create positive outcomes and a seamless care journey. This position is embedded in Woodhaven Pediatrics and follows a Monday to Friday schedule - no evenings, weekends, or holidays are required. Key Responsibilities: Utilize both virtual and in-person methods to coordinate care with patients' medical providers and other necessary mental health professionals. Conduct screenings and assessments for a range of mental health and substance use disorders. Monitor treatment progress and changes in patients' clinical conditions actively. Assist in psychotropic medication management, focusing on adherence monitoring and effectiveness. Deliver brief evidence-based interventions tailored to individual patient needs. Engage in regular consultations with psychiatric consultants to optimize treatment options. Create and implement relapse prevention plans for patients nearing discharge from care. Required Qualifications: Current licensure in Michigan as a: Licensed Master Social Worker (LMSW) Licensed Professional Counselor (LPC) Licensed Marriage and Family Therapist (LMFT)
Experience in providing behavioral health services to pediatric patients. A minimum of 3 years treating behavioral health conditions. At least 1 year of experience in integrated primary care settings. Background in using evidence-based psychosocial treatments. Eligibility to enroll as a provider in the Medicare program. Preferred Qualifications: Experience in the Collaborative Care Model (CoCM). Exceptional organizational and follow-up skills. Ability to maintain professional relationships with patients and care team members. Experience working with patients in both in-person and virtual formats. Comfort with technology solutions and adaptability to new tools. Compensation for this position ranges from $58,800 to $105,000 annually, depending on various factors, including local labor markets and relevant experience.
