Now Hiring: MDS Coordinator RN – Grand Rapids, MI

Are you an experienced RN passionate about clinical excellence, team leadership, and ensuring accurate reimbursement? We’re seeking a skilled MDS Coordinator RN to lead the Assessment Instrument (RAI) process at our sub-acute rehab facility in Grand Rapids, Michigan .

Position Highlights: As the MDS Coordinator RN, you will:

Oversee and manage the MDS/RAI process to ensure accurate, timely assessments.

Supervise a team of MDS Nurses—conduct evaluations, mentor, coach, and schedule.

Lead interdisciplinary care planning and coordinate Medicare/insurance utilization efforts.

Review and analyze clinical data to optimize PDPM reimbursement categories.

Educate staff on documentation best practices for patient care and compliance.

Participate in Triple Check, QAPI, and facility leadership initiatives.

What We're Looking For:

Current RN license in Michigan (required)

Graduate of an accredited School of Nursing

CPR certification

Minimum 2 years of nursing experience, preferably in a rehab or skilled setting

Prior MDS experience and PointClickCare proficiency

Familiarity with CMS RAI guidelines strongly

Strong communication, organizational, and leadership skills
